Reclaim for supplementary allowance

Hello, My mother in law has just recieved a form to reclaim for supplementary allowance. We did not ask for this form and are confused as to why we should fill it in and send back. She has her widows con pension and also has an allowance for taxis back and forward to the local hospital , Has anyone filled in one of these forms before and what are they for, Sorry if I seem naive but we are only adjusting to taking care of my wifes mother and as far as forms go we are not up to speed, Any help would be appreciated.

It would not be usual to receive a form for Supplementary Welfare Allowance in the post unless the person is in receipt of Rent Supplement or Mortgage Interest Supplement (which is subject to regular review).

It may have something to do with the allowance for taxi fares, if that is paid via the Community Welfare Officer. Is there a return address or contact number on the form? I'd suggest contacting the sender to clarify exactly what the form's all about.

Gipimann thanks for your response, I should have just picked up the phone in the first place but wanted a heads up on what the form was for beforehand. Taxi allowance and CWO seems to make sense, Thanks again Gipimann.
