Sorry about this , but does anyone know if I have to get the form for a Medical and bring it to the Doctor for him to fill out or will the doctor have the form?
It's for my Father who is 82 . Renewal of his drivers licence.
A while back when I asked my G.P. about completing a particular medical form in connection with driving licence they did not have it, could not print it off and I had to return the following week with the completed form for their signature and stamping. So moral of the story is bring one along.

Where a medical report form is needed, a registered medical practitioner should complete your medical examination and complete the form. You must sign the declaration on the medical report form in the presence of the registered medical practitioner. It must be submitted within 1 month of the medical examination.
