Sorry about this , but does anyone know if I have to get the form for a Medical and bring it to the Doctor for him to fill out or will the doctor have the form?

It's for my Father who is 82 . Renewal of his drivers licence.

Thank you.

Hi,

A while back when I asked my G.P. about completing a particular medical form in connection with driving licence they did not have it, could not print it off and I had to return the following week with the completed form for their signature and stamping. So moral of the story is bring one along.

Details of what's required is on Citizens' Information site here. Form can be downloaded and printed from there or pick up copy from nearest NDLS office by the looks of things.

From the Citizens' Information site:

If you are 75 or over, you will need a certification of fitness to drive from your doctor to apply for a 3-year or a 1-year licence. You can read the https://www.ndls.ie/images/PDF_Documents/NDLS_Sla%CC%81inte_&_Tioma%CC%81int_2022_WEB.pdf (medical fitness to drive guidelines (pdf)).

You can https://www.ndls.ie/images/Documents/Forms/Medical_Report_D_501.pdf (download the medical report form (pdf)) (it must be printed back to back on one page). You can also get a copy of the form from your local NDLS centre.

Where a medical report form is needed, a registered medical practitioner should complete your medical examination and complete the form. You must sign the declaration on the medical report form in the presence of the registered medical practitioner. It must be submitted within 1 month of the medical examination.

Thank you so much for your help.
I will get it printed from the website tomorrow.
Ease my mind about it all..
Thanks again. I really appreciate it.
