Does my employer, by law, have to provide Permanent Health Insurance on my behalf if I am in a company pension scheme?
 
+1 what MaryBM said. Your employer has no obligation to provide health insurance by law. Your contract, however, may state differently.
 
There's a difference between Permanent Health Insurance (usually known as a disability policy) and Private Health Insurance (such as VHI). OP, you are correct, some pension schemes have an "add on" for Permanent Health Insurance (or a death-in-service benefit) and some do not. An employer is not obliged to offer either PHI or private health insurance.
