Newbie ETF Questions

I've just finished reading the book The Millionaire Teacher and I think its class - such good, common, sense info. I want to follow the plan in the book:

Set up 3 index funds - US stocks, World Stocks, World Bonds
Pay $200 a month and keep my portfolio balanced
I am 32 and investing long term.


I've spent a good amount of time on here reading up on ETFs but still have a few questions:


1. Should I pick an Irish/EU domiciled one and pay the 41% exit tax every 8 years or go US domiciled?

2. If I put money in monthly will I have to pay exit tax monthly in 8 years time, or is it pay exit tax on the total 8 years after starting the ETF?

3. Which ETFs would people reccomend for low TERs and wide diversification?

4. What is the the cheapest way of buying an ETF. Online broker vs financial adviser?

5. How much do Saxobank charge? Is it just the TER on the ETF and €12 transaction fees or are there other fees?

6. Can you buy Vanguard and iShares from Saxobank?

7. Are there alternatives to Saxobank? Keytrade.com seem to get mentioned a lot.

8. Is tax form 11 difficult to fill out. Can I do it myself or do I need to go to an accountant? (I have no financial experience)
