Last minute avc and form 11 returns

I did a last minute avc prior to retiring from the public service last year.How do I deal with this in my form 11 returns for 2015?
The life assurance company name appears in my pre populated form and even though I gained nothing, apart from a tax rebate, do I still have to pay something?

ROS keeps telling me that details must be provided for each employment, so is the life assurance company also treated as an employer for tax purposes?
Any help would be appreciated as I'm utterly confused.
 
I had one of these.

I sent a copy of the tax certificate, which you should have received from your AVC provider, to Revenue with a note stating that it related to a single premium contribution of [amount] euro to the [name] AVC scheme paid in [year] for which relief has not already been granted and for which relief is being claimed for [year].

The amount then ended up as a tax credit for that year in my account in PAYE Anytime. I never used a form 11.
