Hi folks

My 18 year old daughter is going to USA at the end of June. She is going for 2 weeks and will stay with my brother who is there (legally) over 20 years. I just want to check that all the documentation is in order so that there are no unexpected surprises at the airport prior to boarding the plane. Belt and braces approach here, I'm afraid.
I have sought entry for her under the Visa Waiver Programme (VWP). My understanding here is that if
(i) she is a citizen of Ireland
(ii) has a machine readable Passport
(iii) travelling for pleasure/holiday
(iv) staying less than 90 days
(v) is holding a return ticket
(vi) travelling with recognized airline DELTA
(vii) has completed the on-line ESTA registration and received approval (reference number) for Visa waiver travel.
No previous deportation, criminal record, diseases or overstaying visa.
Do you get form I-94W from the airline or at the airport?
She is not working (J1), studying or anything like that.
Does anyone know if there anything further required?

That form no longer exists.

All is needed is the passport and a completed customs declaration.
 
I-94 information is automatically gathered from your travel information when booking your flight etc, I just came back to the US from Rome and the only forms handed out on the flight were Customs declarations, one per family.

Although you will most likely clear customs through Dublin if departing from there. The check-in counter will give you a customs form with your gate pass.
