The correct application form for the renewal of a British passport

I have been asked by a neighbour who is a British citizen and who wishes to renew his British passport to download the correct form for him. I do not want to make a mistake and I would like if someone can give me the correct Form No and link.
Would appreciate any help

Here is the link. Things have changed recently and although you can send the application to Merrion Road ~ British Embassy there ~ I can't find out the price in euro and they will no longer accept walk-ins.

You now have to do a postal application and send it to Belfast. The embassy doesn't have anything to do with passports any more.
