Yes - backdated claims are being limited to 4 years instead of 10 as is the case now. See [broken link removed].

No - it's whatever tax office is mentioned on your statement of tax credits or other communication from
Revenue.

Capital Taxes and perhaps other departments but not
PAYE (I'm assuming that you're an employee and not self employed).
Do I have to fill out a return for each year ?
I presume so - at least
MED1 forms for each year, not necessarily full
Form 12 returns unless your other tax affairs are out of date. Include copies of your
P60s for the relevant years too and perhaps a note explaining what you're looking for.

MED1/2 forms are available [broken link removed]. If you can't find a specific form for a specific year then it doesn't really matter - just make it clear which form is for which year (e.g. cross out the printed year and write in the relevant year).
Does everything have to be done and dusted to obtain tax back by the 31st or whenever tax office shut or just register.
For any year up to 2000 I presume so since after December 31st 2004 you will only be able to claim in respect of 2001 or later presumably? For 2001 onwards there is no rush just yet. For 2004 you probably won't be able to claim medical expenses relief until you get your 2004
P60 in early 2005

See . In my experience a claim form is not absolutely necessary and a letter containg clear details of the contributions and years in respect of which you are claiming tax relief is sufficient. Don't forget to include copies of any
RAC (
Retirement Annuity Contract - aka
Personal Pension Plan) or
PRSA1 certificates as proof of contributions made. Again copies of
P60s are also essential.
