Either the normal Form 12 tax return that you would normally use to declare extra income or claim tax relief on medical expenses.


Or a Form 11, if you are a self-assessed income taxpayer.
 
I didn't think that these forms existed anymore and practically everything is done online via myAccount or ROS for PAYE and self-assessed taxpayers respectively?
 
They still exist in paper form and could be done on paper (which is much more complicated for most people).

I fill it in online on ROS - it's still described as Form 11
