Tax credit form for work uniform

Would anyone know where I can get the revenue form for claiming tax credits for a work uniform. I looked on the revenue site but I couldn't find it. I need to get a hard copy for this person (not through reach services). BTW its for a nurse.

If your a college lecturer you get €518 or a kitchen porter you get €21.

Edit to say you need to just ring the 1890 number, no claim form apart from that form when you start first time employment and you state your occupation. The expense is available at the high rate so 6 euro a week to the nurse.

thanks Davidoco. The girl knows how much she has to claim as I'd already given her this excel sheet. Its for the last 4 years also. Does she just ring revenue then, was your post cut off

You can ring 1890 333 425 and quote your PPS number. Say you work as a nurse and you want your tax credits to inc flat rate expenses and you would like them backdated for 4 years.
