Redundancy Waiver & 25% Tax free Pension lump sum

I was made redundant 10 years ago and at the time I recall there being a waiver that gave me a bigger redundancy payment but would impact my 25% tax free lump sum on retirement

This was a DB scheme and I had 120k in AVCs with this employer.

I cannot recall if I signed this waiver or not.

Two questions:

How do I check?

Does this affect other pension pots outside of that particular employment?

I dont know how to check but the waiver only applies to the affected employment. Other pots from other employments are not affected.
