Recommend good form-creation software

Anyone know of good software for creating online forms? The requirements would be as follows: -

  1. The form would need to be able to make completion of certain fields obligatory, i.e. stop proceeding if an end-user tries to skip obligatory fields.
  2. There should be a limited level of interactivity possible. For example, if the answer to Question 3 is "No", the form skips straight to Question 4, but if the answer to Question 3 is "Yes", a few sub-questions are brought up (which remain hidden otherwise).
  3. At the end of the process, the form should generate a PDF (or other printable document) which can be printed off locally by the end-user, pre-populated with their replies.

Have you any good examples where this is being done on some other website?

If so, you could call up the company and ask them how they did it. It should be a relatively simple job for a web designer.
