Reclaiming excess tax paid last year

Im sorry if this question has been answered here before but I can't find it anywhere.
I paid excessive tax last year and Im wondering how I go about claiming it back? I have my P60 and P45, is there another for I need to fill out?

You can file on PAYE anytime on ROS but if youo want to file on paper the 2008 tax return is Form 12. Ensure that you also have details of any allowable items like refuse, unreimbursed medical expenses etc. as they may help boost any refund due.
