Questions on RTB Notice of Rent review form

Hi,
I would greatly appreciate any assistance with filling out this form -specifically part G declaration by Landlord that all information is correct. There is a space to sign. Then there is `or` and space for a signature , name block capitals and date signed and served.

1) do I sign both
2) date signed and served - I am registering and posting outside country - it will take minimum 7 - 10 days to arrive - what date do I put on the declaration - ? the day I post it
3) Could I scan form to email and get a family member to print off and post/hand deliver? would that be valid? I am allowing plenty of time for the notice to arrive.

I really don`t want to mess this up.

Seems obvious to me.
You sign one of the fields depending on whether you're the landlord or the landlord's agent, and you enter the date that you sign it.

Thank you, yes that part does seem obvious....NowI couldn`t see the printed detail on the signature box!

Is it ok to send a scanned copy of the form by post then?
