Just write to Revenue with details of the unclaimed credits/allowances and including a copy of your P60. That should do it or else they may ask you for further clarifying info or a Form 12 return for the year. It's relatively painless in my experience to backdate claims for for credits/allowances/reliefs. You can do this for up to 4 previous tax years. If you are registered for PAYE Self Service Online then you may be able to do this stuff online too.

For mortgage interest tax relief it may be credited to your lender thus reducing your mortgage costs. For the others you will probably get a refund by cheque/bank lodgement.

In my experience if there is no obvious form - or even if there is - simply writing to them with the necessary details works.
