It's always the easiest option to try and get the P45 off the employer. Threaten that if he does not give it to you, then you will go th Revenue. If this doesnt work, then take Clubman's advice and contact Revenue directly.
I couldnt get a p45 off a previous temporary job before and I just rang the tax office and they sorted it out over the phone as I had the old and new employers tax numbers

From the sound of this, I suspect that the employer may genuinely be confusing P60s with P45s.
2 options
1. Go back to employer and ask them for p60 or p45. If confusion persists, ask them to get the pay & tax details from their accountant or payroll person.
2. Get details directly from Revenue by ringing the local paye helpline.
I had similar issue with previous employer whom I felt did not have me on the books, asked asked and asked again, getting nowhere, finally phoned the revenue and lo behold the next week a letter of apology and my documents from the previous employer
If employer refused to send you a p45 contact the revenue, I actually had a phone call from Revenue office in LImerick today about one that hasn't been issued from here yet since last september..
