Out Sick from Work: Claim forms?

Hi All,

Apologies if posted in the incorrect forum.

I've been quite unlucky with the first trimester of my pregnancy and have been signed out sick after contracting Bronchitis, Food Poisoning and a very nasty virus, not once but twice all within the space of about 7 weeks. My annual sick leave covered the first two bouts, a total of 5 days. Between the most recent two periods (Virus both times), its a total of 8 business days unpaid - the first was 5 bus. days, the second bout was 3.

My doctor mentioned claim forms as this amount of sick leave is unpaid. Can anyone advise me further, as I received no forms from my doctor and have never had to do this before. I'm usually a very healthy person so I can only assume I've been more suceptible due to the pregnancy.

Any / all help appreciated, and I'd be interested to know if the appropriate forms were downloadable.
 
You get the form (MC1) from your GP.
It isn't available online as the doctor has to fill in part of it.
 
Thanks Berni, she mentioned it on my visit but never gave me a form when I left, hence the confusion. I'll make a call
 
The MC1 application is attached to the Medical Certificate which your GP should have given you. It's not a separate form.
