Hope this is the correct forum to post in. Does anybody know, or has anybody (having shown proof as been next of kin), been successful in applying for nursing and medical notes of a deceased person.

should be able to do this through the consumer affairs office (freedom of information) of your hse area yourself or via your solicitor..

you must apply to the correct hospital under the freedom of Info acts 1997 & 2003 to do so. you must be able to show proof of your relationship with this deceased person, ie if you are a daughter/ son your birth certificate will reflect this, you must also submit a copy of your own identity such as driving licence or passport. be realistic in what you are requesting, try to keep it to the episode you are interested as it can take some time to copy charts that are years old that you really dont require. but yes you are entitled to obtain same

Well thats what I thought too. The hospital involved actually issued me with the appropriate forms with provision on that form for next of kin to sign. Supplied all the relevant proofs etc. Two days later they call to say I was given incorrect information and it would not be possible to give me the records - no specific reason given. I wasn't looking for heaps of info - the patient was only in the hospital for less than two days.

to answer your question could you give me your relationship to the patient, you may pm me if you want
