Medical card online, student- what docs?

I have just recently obtained a medical card and (after a few attempts) have completed my teenage daughters application online.

She is single, with no dependents and is dependent on me. She hasn't worked and is full time in school.

Does anyone know what documentation may be required to send in? And are original docs or photocopies required? Mentions sending in P45 for proof of PPS number, but sure she wouldn't have that.

Any info would be greatly appreciated as have been trying to sort this for a few weeks.

originals are usually required and will be returned. Any one of these would do.

PPS Numbers are printed on the following documents:

• Letter of Notification
• Social Services Card
• Drugs Payment Card
• P60
• P45
• Tax Assessment
• PAYE Notice of Tax Credits
• Temporary Payment Card.

Hi, thanks for the info. She has never worked so no P45/P60. Will have to look and see if I have an old drug payment card with her info on it, but if not I don't know what would do.

If all fails and you cant dig up a pps/rsi number just attach a note with the form you are sending off explaining the situation or even ring them might be quicker.
