two years ago my work hours were cut from 35hours to 20hours a week i applied for medical/gp card and was granted a medical card (I am single and have a mortgage).

I was reviewed last year, my work hours had increased to 30hours a week at that point and slightly above the income threshold, i gave all relevant documents including payslips and was given my second medical card.

I have now recieved a declaration form asking if my circumstances have changed (i was expecting a review form like last year). Im still on 30hours a week and still a bit above the income threshold, i still have my mortgage and expenses which im struggling to pay.

My question is should i just sign the form saying nothing has changed or ring them and ask for a review form.

I understand from your post that you have been earning above the income threshold for over a year now. So your circumstances have changed (initial card was given when you were on 20h) and you may not be untitled to a medical card anymore.

I admitted to being above the threshold in last years review and included 2 months payslips with the review form.

My circumstances havnt changed from last year (im still on 30hrs and mortgage payments have been tough to meet)

This is why i asked.

thanks for the answers folks...think ill just sign the form and if they want something from me they can ask
