two years ago my work hours were cut from 35hours to 20hours a week i applied for medical/gp card and was granted a medical card (I am single and have a mortgage).
I was reviewed last year, my work hours had increased to 30hours a week at that point and slightly above the income threshold, i gave all relevant documents including payslips and was given my second medical card.
I have now recieved a declaration form asking if my circumstances have changed (i was expecting a review form like last year). Im still on 30hours a week and still a bit above the income threshold, i still have my mortgage and expenses which im struggling to pay.
My question is should i just sign the form saying nothing has changed or ring them and ask for a review form.
