Med 1 and 2 enclosures

Hello, just filling out 4 years worth of med 1 and 2 forms using hard copy forms printed off months ago and wondering have they changed the forms again?

Could have sworn I had to include my P60 and husband's tax clearance cert? If so, do they have to be photocopies or originals? Just no mention of the tax clearance cert when I checked the form just now?

Also are the hospital booking fee for semi private maternity care and the scan fees allowable?

you do not need to send anything with the MED 1 forms, you fill out your Gross Pay for the year and the Tax Deducted (from p60)
