I was on maternity leave in 2004. I took additional unpaid leave which meant that I did not receive pay in 2004 after my maternity leave ceased.
What do I need to do to get my tax refund?
I do not pay full PRSI but I do pay a reduced rate PRSI.
Are there any other refunds due to me?
Send a form 12 with your P60 to your tax office and you will get your refund. If you have stopped working send a P50 with your P45 to your tax office. A P 60 is given to you by your employer at the end of the calendar year with details of your gross earnings and tax deducted. A P 45 is what you are given when you finish a job. A P50 and form 12 are available on the Revenue website.
