Quick question, what do you need to ask your employer for when leaving a job. P45, P60, reference, holiday pay?

Your P60 is only issued once a year, so you will not receive it on leaving your job.

Occupational pension options form.
Make sure that you have P60s and payslips from previous years with that employer.
Gold watch if you've been there a while.
