Is there only one option here, namely to leave it to one of the main charities or is
there a way to target it to a particular project so that you know your money
is doing most good.

Example: Funding raising for a high tech scanner or special unit to be built etc


Any other options that would ensure you money is being well spent.
 
Yes, that is possible. Charities get a lot of restricted or targetted donations and a well run charity will have a finance and fundraising system designed to track donations in this way.

If course, with a large donation, you should do your research carefully and word the will correctly.
 
Unless you are about to die in the immediate future just leave it generally. Their needs may have changed. But you can attach a letter giving guidance which they might or might not follow.

It might also be helpful to nominate alternative charities or bequest options in the event that the charity no longer exists or has changed hands at the time of your demise. This would ensure that no unintended bequests are made if they would not have been your wish.
