Health Insurance Laya - shortfall on Ophthalmic procedures

I see Laya are introducing a shortfall of 20% on certain ophthalmic procedures on some plans.

I cant get a straight answer frm Laya as to what exact procedures attract this shortfall...

anyone any ideas of the procedures?

Most definitely cataract procedure and possibly injections of the eye
