JSB when leaving Job whats required

Hi I had to leave my job as they could no longer offer me a flexible working arrangement. I assume I need to supply the social welfare office with a letter to this effect but do you know how it should be worded and what other details I require to claim the benefit. Also Do you know if I will receive this benefit straght away.Also can you state the hours you need to work when looking for a job and the location of the job (within reason).
