Intermittent problems with UB visa debit card

Has anyone else with one of the new UB visa debit cards had any problems with it? I find that regularly when I go to an ATM machine it rejects the card once or twice before accepting it. The same when paying at a till in a shop - usually takes a few attempts before it is accepted.
 
I've been using mine since February and have never had a problem with it at all
 
Thanks folks, rang UB just now to order a replacement card. They said everything looked fine and the card isn't scratched or anything so not sure why it was acting up but easiest thing is to get a replacement.
