Income tax return - which form needed?

I need to submit an income tax return for my late father for Jan up to date of death. What form do I need and is it available for downloading?
 
Either Form 12 if his income was predominantely PAYE (e.g. ARF pension) and he had less than €5000 non PAYE income otherwise it is Form 11 if he is a chargeable person. Both can be downloaded from the Revenue.ie website. You may need to wait until next January though for the 2024 forms to be available.
 
Dad had a state pension and very small UK pension so is it 11 or 12 form that is required?
 
You need to complete a Form 12S.

Revenue will only accept this in paper format for a deceased person.

You can upload a PDF copy of this via MyEnquiries.

Beware, they may take an inordinate time to process it. And you will probably need to follow this up with at least one phone call.
