Does anyone know what address I write to to get my medical assessors report- I went to a review as Im on illness benefit, it wasn't my normal doctor, and as a result of his report my benefit has been stopped. I am appealing this and know I can get the report under the Freedom of Information Acts but not sure which department I send the request to. Can anyone help me with this please? Thanks
That address is out of date, as the Townsend Street building closed down at the end of May 2010.
To make an FOI request, you can use the form available on the welfare.ie website (link below) - it includes the address where the request should be sent.
