How to log your stock trading

I'm trying to find the most tax efficient way of logging my purchases and sales of various stocks
I guess a table in Microsoft excel would be best. Any ideas of how this table should look, what headings I should use etc.....
Perhaps there is an app out there that can do all this ?

I use Excel and the headings I have are:
Share Transaction Transaction Date No. of Total Broker Stamp Total Transaction Effective Other Transaction
Type price executed shares investment charge duty Costs value share price details cost % / Notes

I then use a different worksheet to record the current value of the holdings and other notes.
