How to go about getting EB license (car & trailer)

Hi guys

Have a full B license at the moment but would like now to be able to tow a large trailer - how do I go about applying for this license, do I fill out a provisional form again or is there another form for it.

Also I am just wondering, if you are applying for a provisional for EB can you basically tick all categories and have one provisional for all?

Some of the trailer categories require you to supply a medical report (not EB), so you'll need that if you're going to tick more than one category.
