How to apply for an off sales License

I was wondering if someone could point me in the right direction and provide me with an idea of how much a license would cost.

I understand the basics.

Apply to the courts, courts grant permission, Revenue issues license.

But what is the form to apply with to the courts, how much does this cost, what do you need to provide with the application (am sure this will be on the application form) etc.
