How is safety at our sports stadia managed?

Mod Edit:the following general question arose out of a specific discussion in Letting Off Steam. Please use this thread to discuss how safety is managed at large sports stadia in general and use the LOS thread to discuss the particulars of Pairc Ui Chaoimh or other specific stadia.

• Who issues the safety cert for <.......>? Is it the Gardai? The Local Authority? The Fire Department? Why is <.........> issued a safety cert (if indeed one is issued) when there have been constant crowd problems whenever a big game is played there?

It may not be fully clear above so as a further question....

Does anyone know who I might issue a formal complaint to with regard to health and safety at sporting events? I intent raising one after the Cork Vs Kerry game in Pairc Ui Chaoimh yesterday.
Any help or guidance would be appreciated.
