Your local Garda station will have a list.

Any solicitors practice should have an authorised Commissioner onboard. Try your local solicitors office. THey usually charge a small fee (I was charged €10).

Every practising solicitor can sign the form and €10 plus €2 per exhibit is standard - you should NOT be charged more.
