How do i claim tax relief on income protection policy

Hi looking for some help please.

I was going through some insurance policies I have and noticed on my Income Protection Policy that i can claim tax relief on my monthly premium? I have had this policy for nearly 2 years and never claimed this? Has anyone any advice on where I can claim this relief, I have tried to contact my insurance broker but the person dealing with it is away for 2 weeks. Can i back date my claim?
 
Get a statement from Income Protection provider and post it into your local tax office and request them to deal with refunds for prior years
 
Is there any tax form to go with this statement or do you just send in the statement on its own?

No apparently its only taken at source if you go through a group/union/work scheme. If you go individual then you have to claim the tax relief yourself.


On another question does anybody know has the tax relief been reduced to just the marginal rate on these policies?
