Help filling in Form12

Hi, I have always had my tax returns prepared by an accountant friend for a few quid. He is no longer available so I'm starting to do it for myself.

I can't find any document to explain how to complete a FORM12 PAYE tax return form.
I have 90% of it and my MED 1 completed but need a few sections clarified and I really want to learn to do it for myself. Any suggestions on where to get self help?
 
Thanks Statler but I looked at that and it's more for non PAYE and doesn't provide a guide to completing FORM12.

I have very basic novice questions like
1 - Is it my details that go in the Return Address Secion on page 1?
2 - Gross Amount of Income, Q3 page 2, is that my P60 figure or the total from my Dec payslip? P60 is a few thousand lower.
3 - Q16 page 6 - I have a current account but would have made probably in the order of €20 MAX in interest for the year. I have no idea how much i actually got. Do I need to worry about this?
4 - I received a tax free mileage allowance, does this need to go anywhere?
5 - I also received a tax free food allowance, does that need to go anywhere?
6 - My employer pays a contribution to my health insurance but I get it as an extra taxed income each month. Do I need to put that in Q50 page 12 OR is that only if the payment was made directly to the insurance company by my employer (BIK issue).


Any help out there?
