Gravel Patio area. How deep to dig?

I am going to put a gravel patio area in my garden. Somewhere just to put a table and chairs and BBQ. The area will be about 10' x 12'. Do I just dig out the existing grass, lay a foundation of sand (what depth) and then pour the gravel on top of the sand (again what depth for the gravel). Do I need to put in some sort of cover to stop weeds growing through? Would this go down first or on top of the sand? I will run a border of granite type blocks around the perimeter.

I have an old carpet to dispose of. Would that do to stop weeds?
