FS Ombudsman online form compulsory information comfusion

I wanted to file a complaint online but the three asterixed boxes under "Complaint" have me confused. As they are compulsory, I expect the form won't submit without the information it needs but what do I put there? Does every box need input or is it just the one, be it "account", "policy number" etc?
