Forms?

Hi,
Im trying to get onto the scheme here in limerick and i have a question about the forms.
I rangs limerick coco and a girl there emailed me two forms.

One of the forms wants my latest P60. Any idea's where i can request this? I moved house recently and seem to have mislaid the one i got in the post.


The other form wants my "Income Tax REference number" I have no idea what this is? Any ideas? Is it my PPS number?

You can get another copy of your P60 from your place of work or you can call revenue
