form 12, invoices

i am a paye worker and i have done 3 jobs freelance jobs (durin 2004),
is the 31st deadline relevant to me or is it just self employed

form 12 asks for p60 i would not have that yet so surely i cannot do my return until the end of the year?

the return due now is for 2003.
you make your 2004 return next Oct

quick question ....

sorry to add a sideways question to this, but is form 12 still available to download from revenue.ie ?
I downloaded it last year and filled one out but i'll be damned if i can find one this year on their website !
maybe its just me or is this part of the revenue revamp and update of site that this form cannot be downloaded no more ??????
