Form 12 (HELP PLEASE)

Hi, I am in the middle of trying to fill in Form 12 going back to 2005 and I have a couple of Questions if somebody would please help - I am going mad! On page 10 question 36 (2008) they ask about RACs could someone tell me if this is a personal pension taken out independently of your employer? and maybe talk me through filling this section our - would really appreciate any help
 
Personal Pensions (also known as RACs – Retirement Annuity Contracts)

A Retirement Annuity Contract “RAC” is the formal name for what is normally called a personal pension. An RAC is a particular type of insurance contract approved by the Revenue. An RAC is a defined contribution pension plan. The value of the ultimate benefits payable from the contract depends on the level of contributions paid, the investment return achieved and the cost of buying the benefits.
