Form 12 - Employee Share Participation Scheme

I am completing a Form 12 for 2007. From my workplace share participation scheme I have a Tax voucher indicating the amount of dividends paid. US Tax and Irish Tax have been deducted from the dividend prior to payment. Can anyone advise under which section (if any-ie as 20% Irish Tax has already been deducted) I should declare these dividends.
