I have two PAYE jobs, never needed to submit a tax return before

I bought shares on the ISE in 2007 and got a dividend of about E 250 in 2007

Not sure how to tell revenue about this dividend income? Their website has two forms
Form 11
Form 12

Form 12 has less pages! and refers to "employees"

However in section 3 there is only one space for my PAYE employers name, and I have two PAYE employers, also I can't see where I mention my Irish dividend income? also should I mention my tuition fees on this form again, even though I already told revenue about them via a single form in 2007.

I'd really appreacite any help here.
 
As far as I know you only need to return a Form 11 if you have non PAYE income over a certain limit (c. €3K if I recall correctly). If Form 12 doesn't have space for all the necessary details then perhaps you could just attach additional info on a separate sheet? If this is a problem Revenue will soon get back to you. But they are normally pretty pragmatic in such situations in my experience. For example I often just wrote to them with updates on my income/tax situation rather than filling in the relevant forms and they always dealt with the matters arising.

Isn't Form 12 section 18 the one for dividend income from Irish based companies?

If you are doing a return for the year in which tuition fees were paid and you have already mentioned these to Revenue you should still put the info into the Form 12. The Form 12 should contain ALL information relevant to the year even if already notified.

Then you should complete a Form 12.

Vector, if you just have €250 dividend income that you need to declare then you can phone your tax office as Clubman suggests. However, since you have two paye employers and are claiming tuition fees, it might be worth your while filling out the Form 12 to make sure that your tax has been calculated correctly and that you are claiming all the reliefs you are entitled to.

Also, you should note that you can claim tax relief for refuse charges, medical expenses (not all dental expenses though, only those for which you have a med 2), Income protection insurance premia, contributions (on or before 31 October 2008) to PRSA pension accounts etc, Business Expansion Scheme and Film investments made in 2007.

There are a range of items really.

You should also ensure that, if you sold any assets during 2007, you declare details of all sales etc and the gain.

The tax on gains from 1 Jan 08 to 30 Sept 2008 should be paid by 31 October 2008.
