Form 11 or form 12?

Hello,
I was self employed for a period in 02 while awaiting the incorporation of my limited company. Since then I have been the sole employee of the company and my PAYE earnings from the company are my sole source of income. So my question is so I send in a Form 11 or a Form 12 for directors as I am the director of the limited company?

Form 12 for Directors will suffice. However you may complete a Form 11 if you wish (unlike PAYE employees who are not directors)
