Foreign ATM withdrawls

Hey anyone know the best i.e the cheapest, card one can have for making withdrawls outside the Eurozone?
Looking up on a few the banks websites the rates seem to vary a good deal.
Last year living in Singapore, Bank of Ireland were charging me a minimum of €3 and up to €6.35 say on a €200 withdrawl.
How can the banks get away with this, are Consumer Watchdogs doing anything about it.
Plus when one makes a withdrawl does this fee include a commission for the foreign exchange?
The Single European Payments Act (SEPA) covers some regions outside the Eurozone so why not places further afield?

I presume that the main options would be credit card (either cash withdrawal from a preloaded a/c or cash advances from an "empty" a/c) or the likes of Cirrus/Maestro/Plus+? I suspect that cash withdrawals from a preloaded CC would be the cheapest but you'd need to check the specific charges levied by your bank/cards to be sure.
