Health Insurance First time insurance for 80 year old

First time poster,long time reader!
My 80 y/o grandmother has never had health insurance. It’s a minefield in terms of options.I understand the 2% loading per year after 34 & that pre existing conditions could not be covered for 5 years.Past history of cardiac issues.Could you recommend any particular plans? I’d really appreciate any guidance.Thank you!
