eviction notice

I am looking for a sample eviction notice. Mytenants are 3 weeks late with their rent so I want to serve an viction notice of 28 days on them. I want to include in the notice that if the arrears are all cleared up within the 28 days then I will extend their lease. Can you also tell me the exact process to follow.
