Entitlements on a 3 day week

Apologies if this has been answered already, i couldnt find it i fit had.

I have read welfarites excellent 'Guide for unemployed people'

and found it a great resource, however it doesnt exactly answer what questions i have.

I have been put on a 3 day week.... my take home has dropped by about 40%..... to 450

I intend to sign on for the other 3 days that i can.

I have looked at the UP1 form and its not clear as to how to fill it out for a 3 day week....

1. do i say im working part time for my previous, and current, employer??

2. do i need a revised P60?? if so how long do i have to be on my new wage to get one??

3. how long do i have to be on a 3 day week before i can claim??

4. what do i need to bring with me??

1. The up1 is a 'catch-all' form that is used for your situation as well as full-time unemployment. Therefore, complete 'last employer' as your current employer, with comment beside about 3-day week.
2. Your P60 cannot be revised. don't understand what you mean by this question.
3. You claim on the first day that you are not working.
4. As per key post, plus a letter from your employer givings details of situation.

2. proof of address
